Position Description
This position will serve as a primary contact for medically needy patients. Responsibilities include processing provider claims, using computer systems to record and report patient information, patient case finding, financial screening, program enrollment facilitation, non-medical counseling, case tracking, and preparing all necessary case documentation. Persons in this position will serve as program spokespeople and representatives to participating healthcare providers, local communities, and local health agencies; therefore, this position requires a positive and professional attitude when working with patients, community leaders, and local health agencies and state agency care workers. Persons in this position will be expected to travel between local health agencies in the fulfillment of their duties and may be asked to make occasional home visits. Work is performed in usual clinic conditions; and requires a high degree of contact with patients and families, clinic staff, and outside support agencies. Travel and the ability to respond to needs during off-duty hours may be required in unusual situations.

Major/Essential Functions
Community outreach for Endocrinology Center of Excellence; prepare, submit, and track patient assistance paperwork for clinics; prepare, submit, and track prior authorizations, referrals, and pre-certifications for patients; track medications for patients on patient assistance; the track "no show" or "canceled" patient appointments to ensure patients are rescheduled with providers promptly; track diabetes patients discharged from hospitals to ensure patients are scheduled with providers in a clinic within a timely manner. Occasional Duties
Triage patient questions or comments on the app; answer clinic phones; other duties as assigned by supervisor
Required Qualifications
Education: A minimum of a High School diploma or equivalent.
Experience: At least three years of experience in a business office environment. Higher education may be substituted on a year-for-year basis.
